Sarah A. Bajorek, PharmD, BCACP, MBA is the Assistant Chief Pharmacy Officer - Ambulatory Care at University of California (UC), Davis Health, and an Assistant Clinical Professor with UC San Francisco’s School of Pharmacy. In this role, she oversees the Ambulatory Care clinical services, including Primary Care, Specialty, Population Health & Accountable Care, and the centralized Refill, Prior Authorization, and Anticoagulation services. These teams provide clinical medication management services under collaborative practice and medication access and financial support. Within Accountable Care, the managed care pharmacists complete medical necessity review for ~42,000 lives and collaborate with key stakeholders who assess risk and coordinate operational logistics for high-cost therapies. She earned her Doctor of Pharmacy degree from UC San Diego (UCSD) Skaggs School of Pharmacy and Pharmaceutical Sciences. She went on to complete a PGY1 Pharmacy Practice Residency, a PGY2 Transitions of Care Residency, and obtain an MBA.
